Neighbourhood Health Centres or equivalent neighbourhood health structures. Our neighbourhood model is still in development and is being shaped in line with the requirements set out in the 10-Year Health Plan and the medium-term planning framework.
Neighbourhood principles are being embedded across the ICB’s strategic and operational planning. The ICB is establishing governance arrangements to support neighbourhood priorities, delivery expectations and assurance, and to clarify the respective roles of the ICB, providers, Primary Care Networks (PCNs), local authorities and voluntary, community, faith and social enterprise (VCSFE) partners.
Neighbourhood working is delivered through multiprofessional teams operating the proactive model of care within each PCN. The intention is to build on this existing model by incorporating
national prevention requirements and embedding personalised care principles at both operational and strategic levels.
Somerset ICB does not currently have any partnership with the local Health Innovation Network in relation to weight management services.

Weight management services form part of our block contract with Somerset Foundation Trust and/or the core primary care contracts and we are unable separate out a funding figure for one specific service.
As an ICB we are currently working through our commissioning intentions which will place long term conditions at the heart of a neighbourhood health offer, but we are not yet in a position to define funding allocations or timelines.
